District Testing Change in Participation 

The purpose of this form is to request a change in participation information for the ACT District Testing program. 

To request a change in your test date, test option, or to cancel participation, complete the information below. A response will be sent to the Requestor’s email address if the change is approved or denied.

THIS FORM IS NOT TO BE USED TO CHANGE A SCHOOL TEST COORDINATOR.

All testing staff changes must be made in PearsonAccessnext. Please contact your district official to change the test coordinator for your school.
