How to Apply

Application Deadline: January 13, 2012

Applications must be submitted online by 5:00 p.m., central time, January 13, 2012.

Recommendation must be submitted online by 11:59 p.m., central time, January 20, 2012.

All supporting documents must be sent to the Iowa City address and postmarked by January 13, 2012.

Step 1

Read these important documents before getting started, and follow the instructions provided. Print them or save to your desktop for future reference.

To view and print the documents as PDF (Adobe Portable Document Format) files, select the PDF icons. If Adobe Reader has been downloaded to your computer, the files will transfer to your browser. If not, get a free copy of the software by selecting the Adobe Reader icon below. (Note: When printing, be sure the "fit to print" or "shrink oversized pages to paper size" option in the print dialog box is NOT checked.)

To view and print the documents using Microsoft Word, select the Microsoft Word icons.

Step 2

Register
If you need assistance creating an account, call 877-789-4228 Monday–Friday, 8:30 a.m. to 5:00 p.m. central time.

Step 3

Take the Eligibility Quiz
After you register, you'll be directed to the eligibility quiz. If you pass the eligibility quiz, the remainder of the application will launch automatically. If you do NOT pass the eligibility quiz, you will receive a notification, and the application will not be available to you.

Step 4

Complete the Application
You will be able to return to the application to update and edit as frequently as desired. After you have submitted the application, or after the January 13 deadline, the application will no longer be available.

Note: When you log out of the Review My Status page, the login screen you see will only take you back to the Review My Status page. Closing the login screen will return you to this How to Apply page.
